How We Care for You

Pediatric Primary Care

Comprehensive primary care for children 1 year and older, including well-child visits and sick visits. Vaccinations are not provided.

If under 18 years of age, must be included with one adult member.

Adults 22 Years – 64 Years

$80.00 / Month

  • In-clinic  & telehealth visits
  • 2 house call visits available per year
  • Same-day or next-day appointments
  • Annual wellness exams & preventive screenings
  • Chronic condition management & medication refills
  • Minor procedures included       (exclusions apply)
  • Lifestyle & wellness coaching

Family Option (2 adults + 2 children)

$280.00 / Month

  • Includes all adult & child membership benefits
  • Additional children: $10/month each
